Have a career, dreams for the future, and make meaningful contributions The Housing Focused Case Manager of the SJCS Ithaca Shelter Program is the primary staff person with regards to housing placements for shelter guests.
The Case Manager will work with residents to secure a safe alternative housing placement or permanent housing, as quickly as possible, and link the individuals with needed supports to sustain the housing placement.
Case managers primary focus is to end homelessness by rapidly returning residents to permanent housing through an individualized housing plan co-developed between the guest and the case manager.
Essential Functions:
Ability to engage and build strong working alliances with individuals experiencing a housing crisis.
Relentlessly show up and be present in guests lives, especially through times of relapse and crisis Assists residents in a manner that demonstrates respect, honesty, non-judgment and empathy.
Complete intakes for all new guests entering SJCS including an intake assessment, initial case plan, releases of information, PHQ -9, suicide risk assessments, and safety plans as needed; Develop individualized housing plans with each participant, along with the participant.
Continually view guests in a holistic manner and support guests attempts to change.
Ability to work in a team environment and independently with minimal supervision.
Assist residents with housing related activities.
Examples include but are not limited to:
applying for apartments, accessing subsidies, developing a budget, setting up utilities, accessing fund for security deposit and first months rent, reviewing lease obligations, and securing furnishings.
Provide linkages to community resources for sustainability including links to medical, mental health, substance abuse, child care, and transportation as needed.
Demonstrates knowledge of available community resources.
Provide assistance as needed during their move into stable housing-such assistance may include accessing rental assistance money, donated furnishings and coordinating with volunteer movers.
Provide housing counseling and budget training and workshops for guests to help prepare them for independent permanent housingworkshops may include such topic as tenant rights and responsibilities, housing discrimination, communicating with landlords, etc.
Maintain documentation of trainings, budgets, housing contacts and other pertinent information in accordance with agency and best practice standards.
Facilitate weekly Community meetings with shelter residents.
Coordinate speakers, in-services, and social events for community meetings.
Follow up with all guests to be assured that their needs are being met and function as a contributing member of a multidisciplinary team with City of Alexandria employees, deployed staff, volunteers and community agencies and assure all relevant information pertaining to guests is disseminated to all appropriate staff members in a timely fashion.
Maintain guest files (in accordance with funding source and program standards established by Federal, State and City regulations).
Maintain guest confidentiality.
Perform other duties as assigned.

Must be dependable and able to report to work in a timely manner and work a flexible schedule outside normal business hours, including weekends and holidays Is approachable, builds rapport well and practices attentive and active listening Supports equal and fair treatment and opportunity for all All CCS positions require that the staff stay awake and remain on site at all times during the shift.

Estimated Salary: $20 to $28 per hour based on qualifications.
